We are writing to share some important news about our medical practice and changes that will be taking place.
After many years of dedicated service, Oak Mill Medical Associates is closed effective October 1, 2025. This number will remain active during regular business hours for a limited time -

We're also pleased to inform you that Dr. Herman and Dr. Weiner are transitioning to a Direct Primary Care/Concierge membership-based practice model as of late summer/early fall. We encourage Dr. Straka's patients to join us. As such, we will no longer be accepting any insurance. This allows us the opportunity to care for our patients as we believe is most appropriate.
This new model allows us to offer personalized and comprehensive care to our patients. We'll be limiting the number of patients to enable less hurried visits, extended appointment times, direct access to your physician via phone and email, and enhanced focus on preventative care and wellness.
We have spent some time considering practice options and believe that, in the current medical environment, we are unable to provide the level of medical care that meets the needs of both patients and physicians.
To better serve you, we are creating and designing a relaxing space on the second floor within the Oak Mill Mall. While we work hard to make this transition as smooth as possible, we appreciate your patience and invite you to learn more about joining our new practice by visiting Harmony Personal Physician's website at
