Dear Valued Patients,

We are writing to share some important news about our medical practice and changes that will be taking place.

After many years of dedicated service, Dr. Straka has decided to retire effective late summer/early fall of 2025. We are immensely grateful for the unwavering care and support she has provided to our patients over the years. Dr. Straka has been a cornerstone of our practice, and we will miss her greatly.

We're also pleased to inform you that Dr. Herman and Dr. Weiner are transitioning to a Direct Primary Care/Concierge membership-based practice model as of late summer/early fall. We encourage Dr. Straka's patients to join us. As such we will no longer be accepting any insurance. This allows us the opportunity to care for our patients as we believe is most appropriate.

This new model allows us to offer personalized and comprehensive care to our patients. We'll be limiting the number of patients to enable less hurried visits, extended appointment times, direct access to your physician via phone and email, and enhanced focus on preventative care and wellness.

We have spent some time considering practice options and feel that in the current medical environment, we are not able to provide the level of medical care that meets patients and physicians needs.

To better serve you, we are creating and designing a relaxing space on the second floor within the Oak Mill Mall. While we work hard on making this transition as smooth as possible, we appreciate your patience and ask you to learn more about joining our new practice by visiting Harmony Personal Physician's website at Visit Harmony Physicians and entering your contact information.
